If you plan to use insurance, in addition to filling out all of your paperwork, please remember to also bring your insurance card with you. Printed documents must be one-sided and on letter-size paper.

• Adults who have scheduled a Holistic/Homeopathic Evaluation ($350.00) must complete New Patient Paperwork (Adult)  & Holistic Evaluation Paperwork (Adult)
